1. Registration Form
2. Parental Sportsmanship Pledge/Code of Conduct
3. One (1) copy of Birth Certificate (not required for In House)
4. One (1) check payable to Berkeley Soccer Association, for Registration Fee (see Schedule of Fees)
5. Full season registrations only: Two (2) refundable $50.00 checks for Fall/Spring Participation Fees, with Parental Participation Agreement
6. Half season registrations only: One (1) refundable $50.00 check for Fall Participation Fee, with Parental Participation Agreement.
