To register via regular mail, please fill out and submit the following items to:

1. Registration Form
2. Parental Sportsmanship Pledge/Code of Conduct
3. One (1) copy of Birth Certificate (not required for the inhouse clinic)
4. Two checks payable to Berkeley Soccer Association; one for the registration fee, and one for $50 for the parental participation bond. Two hours of volunteer time during the season is required to get the $50 check returned to you.
